Maritime Activity Reports, Inc.

November 21, 2014

An explosion occurred aboard a production platform Thursday at West Delta 105, operated by Fieldwood Energy, resulting in one fatality and three injured.

The Bureau of Safety and Environmental Enforcement (BSEE) said it is actively investigating the incident.

As reported to BSEE by the operator, the incident happened during routine maintenance operations on its Echo platform around 3pm CST Thursday.  Contractors with Turnkey Cleaning Services were cleaning a piece of equipment, called a heater treater, at the time of the incident. Damage to the facility is reported to be limited to the heater treater. The platform had been shut in for more than a week and was not producing at the time of this incident.

The operator reports that all three injured were evaluated and released from the hospital and all other personnel aboard the platform are safe.

There has been no reported fire or release of oil or pollution as a result of this incident.

A team of BSEE engineers, inspectors and investigators are currently on the facility conducting an incident investigation. Through the course of the investigation, the BSEE investigation team will interview witnesses and analyze evidence to determine the cause of the explosion.
