Aberdeen Conference Examines Human Risk
Risk management experts from around the world will gather in Aberdeen on March 5 for a conference that will set the agenda for dealing with the issue and impact of human risk in the oil and gas industry. The keynote address will be delivered by Lord Cullen of Whitekirk, who chaired the public enquiry into the Piper Alpha disaster. The outcomes of the enquiry have transformed HSE operational procedures in the North Sea and the offshore oil and gas industry worldwide. Also included in the impressive line up of speakers is David Pritchard, co-chair of the Presidential Technical Commission for the Deepwater Horizon study group (U.S.) – this being the first time a key investigator in the Macondo disaster has publicly addressed the industry in a conference/public setting.
Navy Divers Salvage F-16C Aircraft From GofM
Navy sailors and divers from Mobile Diving and Salvage Unit (MDSU) 2, embarked aboard the Navy's rescue and salvage ship 'USNS Grasp' (T-ARS-51), find and salvage a downed F-16 aircraft off the coast of Virginia. The downed aircraft was one of two F-16 fighter jets from the 113th Wing, D.C. Air National Guard that clipped wings mid-air during a routine training mission 35 miles southeast of Chincoteague, Va., on 1, August 2013. The other aircraft involved in the incident was able to fly back to Joint Base Andrews in Md. without further incident.
Recent Warship Collision Investigators Named
US Fleet Forces Command (USFF) names Rear Adm. Ann Phillips, & Rear Adm. Joseph Tofalo to enquire into 'San Jacinto/Montpelier' collision. U.S. Fleet Forces Command (USFF) assigned a flag officer to lead an administrative investigation and established a safety board to review the facts and circumstances Oct. 15 that led to the collision between the Navy submarine USS Montpelier and Aegis cruiser USS San Jacinto off the coast of Florida Oct. 13. Vice Adm. Michelle Howard, deputy commander of USFF, appointed Rear Adm.
Assessment of USS Hartford and New Orleans
The U.S. Navy submarine and U.S. collided in the Strait of Hormuz March 20, have been undergoing extensive engineering and damage assessments since pulling into Bahrain March 21. Engineering and technical experts arrived in Bahrain to assess the damage to USS Hartford (SSN 768) and USS New Orleans (LPD 18). They will augment Mid-Atlantic Regional Maintenance Center (MARMC) Detachment Bahrain. Twelve Pearl Harbor Naval Shipyard (PHNSY) personnel and two Naval Sea Systems (NAVSEA) experts are assessing the damage to Hartford and New Orleans and have begun initial in-theater repairs.
