Because there are so many different kinds of passenger vessels, the critical topic of passenger safety can sometimes appear as a set of niche topics, each one just distantly connected to another. NTSB Issues Safety Alert on Personal Locator Devices for Mariners

A new safety alert issued by the National Transportation Safety Board (NTSB) urges vessel owners and operators to provide each crewmember with a personal locator device. These devices improve a mariner’s chance of rescue during an emergency.During an emergency at sea, a mariner’s chance of survival decreases if search and rescue cannot quickly and accurately identify their location. Personal locator devices, such as personal locator beacons (PLB) or satellite emergency notification devices (SEND), can accurately pinpoint a person’s location.

Flooded Engine Room Caused Fishing Vessel to Sink -NTSB

Uncontrolled flooding through a hole in the plating beneath the engine room of a fishing vessel led to its sinking in the Gulf of Mexico, the National Transportation Safety Board (NTSB) said Thursday.The commercial fishing vessel Captain Alex was fishing offshore of Galveston, Texas on Nov. 25, 2022, when the vessel began flooding. The four crewmembers on board were unable to stem the flooding and evacuated to a responding U.S. Coast Guard boat. The sinking resulted in an oil sheen and debris field; a reported 17,000 gallons of diesel fuel were on board. There were no injuries.

Gale-force winds and high waves from a storm led to the listing of an elevated liftboat in the Gulf of Mexico, the National Transportation Safety Board (NTSB) said Wednesday. Damage to the vessel and cargo was estimated at $6.9 million.On November 20, 2022, the liftboat L/B Robert was reported to be listing alongside a stationary oil platform in the Gulf of Mexico about 80 miles southeast of Lake Charles, Louisiana. The crew had evacuated two days prior due to forecasted adverse weather in the area, leaving the vessel elevated out of the water and unattended.

A fishing vessel mate not maintaining a proper lookout and conducting maintenance on critical equipment while underway led to the collision of a containership and fishing vessel, the National Transportation Safety Board (NTSB) said Wednesday. No injuries were reported. Undetected corrosion and wear led to an equipment failure on a cargo ship while offloading cargo last year in Houston, the National Transportation Safety Board (NTSB) said.On July 23, 2022, the cargo ship Thorco Basilisk was offloading a wind turbine component at the Greensport Terminal on the Houston Ship Channel when the hoisting wire rope on one of the ship’s cargo cranes failed, causing the component to drop onto the vessel’s cargo hold tween deck.No injuries were reported.

This article examines 10 structural safeguards in Coast Guard Suspension and Revocation (S&R) Proceedings that ensure Coast Guard Administrative Law Judge (ALJ) decisional independence. These safeguards separate potential agency influences from the ALJ decision making process and are critical to mariner due process and fundamental fairness. We will start with an overview of Coast Guard S&R proceedings, followed by brief discussions of the Administrative Procedure Act, Administrative Law Judges, and U.S. Office of Personnel Management oversight.

The National Transportation Safety Board (NTSB) on Thursday cited thermal runaway of a cell within a handheld radio’s lithium-ion battery as the cause of a fire on an oil tanker docked in Baton Rouge, La.The Liberian-registered oil tanker S-Trust was docked at the Genesis Port Allen Terminal on November 13, 2022, when a fire on the bridge was sparked by one of the cells in a lithium-ion battery for an ultra-high-frequency handheld radio exploding, the NTSB's investigation found.

NTSB Urges USCG to Remove Subchapter K Exemptions for Fire Safety

The National Transportation Safety Board (NTSB) recommended Tuesday that engine-room fire detection and fixed fire extinguishing systems be installed in small passenger vessels previously exempted from U.S. Coast Guard regulations in Subchapter K.The NTSB issued the recommendations after investigating an engine room fire aboard the Spirit of Norfolk during a sightseeing cruise near Naval Station Norfolk, Viriginia, on June 7, 2022, with 108 people on board. The fire was too large for the crew to fight, and the passengers and crew were evacuated to other vessels.
