Report: Tanker Didn’t Stop After Trawler Collision
The UK Marine Accident Investigation Branch (MAIB) has released its investigation report into the collision between the crude oil tanker Apache and the stern trawler Serinah (GH 116) in the Firth of Clyde, Scotland on 25 April 2024.At about 1503 on 25 April 2024, the crude oil tanker Apache and the stern trawler Serinah collided in the Firth of Clyde resulting in Serinah sinking rapidly. Pilot Vessel Grounding Linked to Ineffective Use of Electronics
New Zealand’s Transport Accident Investigation Commission (TAIC) has released a report into the grounding of the pilot vessel Takitimu II at Striling Point, Bluff, New Zealand, on December 26, 2024.The South Port’s pilot boat set out from Bluff to transfer a pilot to an incoming ship. After passing Stirling Point the master made an excessive course alteration and at about 18–20 kt the vessel ran aground on rocks and sustained moderate damage to its hull and underwater fixtures. Two of the three people on board suffered minor injuries. There was no pollution.

The UK Marine Accident Investigation Branch (MAIB) has released its report into the death of a pilot while attempting to board Finnhawk from the pilot vessel Humber Saturn, in Humber Estuary on 8 January 2023.The Humber pilot fell from a pilot ladder after likely suffering a cardiac event while boarding the roll-on/roll-off cargo vessel Finnhawk from the pilot vessel Humber Saturn. The pilot hit the deck of Humber Saturn before falling into the water and losing consciousness.
Research Shows Alarm Overload is Undermining Safety at Sea
Analysis of more than 40 million alarm-related events shows most alarms offer little operational value, disrupt rest and push crews toward risky workarounds. New research from Lloyd’s Register (LR) has revealed that excessive and nuisance shipboard alarm systems are routinely overwhelming crews and, in many cases, actively undermining safety at sea. The findings, published today in Effective Alarm Management in the Maritime Industry are based on data collected from 11 operational vessels, spanning over 2,000 days and more than 40 million alarm-related events.
