ATSB: Fatal Pilot Ladder Accident Has Enduring Lessons

Managers of the two vessels involved in a fatal crew transfer accident off Brisbane, Australia, had not ensured personnel had a common and complete understanding of how the transfer would be conducted, an Australian Transport Safety Bureau (ATSB) investigation has concluded.On 9 August 2021, crew were being transferred to and from the bulk carrier Formosabulk Clement via the launch boat PT Transporter in the Port of Brisbane anchorage, about five nautical miles off the coast.The operation involved multiple visits from PT Transporter to transfer crew on and off the anchored bulk carrier.

Lack of Monitoring at Anchor Led to $16.9 Million Accident -NTSB

A bridge team’s lack of effective monitoring of their position while at anchor led to a cargo ship colliding with an anchored bulk carrier and striking a chemical dock on the Lower Mississippi River near New Orleans, according to a National Transportation Safety Board Marine Accident Brief released Thursday.Marine Accident Brief 21/15 details the NTSB’s investigation of the May 8, 2020, collision of the anchored general cargo ship Nomadic Milde with the anchored bulk carrier Atlantic Venus. The Nomadic Milde then struck a nearby chemical dock and grounded on the bank.

Man Overboard: Bulker Crewman Missing off Virginia

The U.S. Coast Guard and other agencies are searching for a missing crewman who fell overboard from an anchored bulk carrier near Cape Charles, Va., Tuesday.   The 33-year-old Chinese crewman reportedly went overboard early Tuesday morning while fastening a ladder on the starboard quarter of the 770-foot Hong Kong flagged cargo ship Lara Venture at the Cape Charles Anchorage.   He was not wearing a life jacket and last seen wearing orange coveralls and a dark blue jacket, the Coast Guard said.   Coast Guard helicopter crews are searching for the missing crewman along with the crews of Cutter Sea Horse from Base Portsmouth and a Virginia pilot vessel.   The Coast Guard has issued an urgent marine information broadcast.

British Columbia: Oil Spill Response shows Unprepared for More Tankers

British Columbian officials on Friday criticized the Canadian government's response to an oil spill in the waters around Vancouver, calling into question plans for new crude oil export pipelines in the Pacific Coast province. Nearly 3,000 liters of oil spilled after an anchored bulk carrier began leaking bunker fuel in English Bay, just west of Vancouver's downtown core, on Wednesday. Officials in the province said the coast guard responded but was slow to contain the slick, which spread towards beaches. They said the federal agency failed to notify the cities of Vancouver and West Vancouver until early Thursday, delaying public safety warnings by more than 12 hours. Latest Weekly ICC Piracy Report

Four piracy and armed robbery incidents are noted in the latest ICC-CCS report. None off the Somali coast. Three robbers armed with knives boarded an anchored bulk carrier via the anchor chain. They attacked the duty crew on the forecastle, who managed to escape and raise the alarm. All crew mustered and proceeded forward. Seeing the alert crew the robbers jumped overboard and escaped in their boat. On investigation it was discovered ship’s stores stolen from the forecastle store. Three robbers in a boat approached and boarded an anchored general cargo ship. Alert crew saw the robbers near the forecastle store and raised alarm. Seeing crew alertness the robbers escaped with stolen ship stores.

Piracy & Armed Robbery: Latest Report

In the past 7-days the following attacks were noted by the International Chamber of Commerce, Commercial Crimes Services. Two robbers armed with long knives boarded an anchored bulk carrier via the anchor chain. Duty crew noticed the robbers and raised the alarm. All crew mustered and rushed to the forecastle. On seeing the crew alertness, the robbers escaped empty handed in their speed boat. About six robbers in a speed boat approached and boarded a barge under two, stole barge properties and escaped. Later the same evening a speedboat with five persons wearing camouflage uniforms approached the tug. Two of the persons boarded the tug and took hostage the duty bosun and held him face down at knife point. Seeing the boarding the Master raised the alarm and all crew mustered.

Piracy, Armed Robbery Update

To date, the following reports were noted by ICC Commercial Crime Services (CCS), the anti-crime arm of the International Chamber of Commerce. 09.01.2013: 0920 UTC: Guyana, South America. Two robbers armed with guns and long knives boarded a berthed LPG tanker from the offshore side by using a grappling hook. They took the C/O and shore security guard as hostage, proceeded to the master’s cabin and stole ship’s cash and escaped. No injury to crew. 08.01.2013: 0750 UTC: Posn: 12:40.2N - 043:16.1E, Bab El Mandeb TSS, Red Sea. About 5-6 persons each in two skiffs doing 14 knots approached a gas carrier ship underway. Alarm raised, anti-piracy measures initiated, distress message was transmitted and the armed security team on board fired a warning flare and showed their weapons.
