Managers of the two vessels involved in a fatal crew transfer accident off Brisbane, Australia, had not ensured personnel had a common and complete understanding of how the transfer would be conducted, an Australian Transport Safety Bureau (ATSB) investigation has concluded.On 9 August 2021, crew were being transferred to and from the bulk carrier Formosabulk Clement via the launch boat PT Transporter in the Port of Brisbane anchorage, about five nautical miles off the coast.The operation involved multiple visits from PT Transporter to transfer crew on and off the anchored bulk carrier.

The U.S. Coast Guard and other agencies are searching for a missing crewman who fell overboard from an anchored bulk carrier near Cape Charles, Va., Tuesday.   The 33-year-old Chinese crewman reportedly went overboard early Tuesday morning while fastening a ladder on the starboard quarter of the 770-foot Hong Kong flagged cargo ship Lara Venture at the Cape Charles Anchorage.   He was not wearing a life jacket and last seen wearing orange coveralls and a dark blue jacket, the Coast Guard said.   Coast Guard helicopter crews are searching for the missing crewman along with the crews of Cutter Sea Horse from Base Portsmouth and a Virginia pilot vessel.   The Coast Guard has issued an urgent marine information broadcast.

Four piracy and armed robbery incidents are noted in the latest ICC-CCS report. None off the Somali coast. Three robbers armed with knives boarded an anchored bulk carrier via the anchor chain. They attacked the duty crew on the forecastle, who managed to escape and raise the alarm. All crew mustered and proceeded forward. Seeing the alert crew the robbers jumped overboard and escaped in their boat. On investigation it was discovered ship’s stores stolen from the forecastle store. Three robbers in a boat approached and boarded an anchored general cargo ship. Alert crew saw the robbers near the forecastle store and raised alarm. Seeing crew alertness the robbers escaped with stolen ship stores.

Two robbers armed with long knives boarded an anchored bulk carrier via the anchor chain. Duty crew noticed the robbers and raised the alarm. All crew mustered and rushed to the forecastle. On seeing the crew alertness, the robbers escaped empty handed in their speed boat. About six robbers in a speed boat approached and boarded a barge under two, stole barge properties and escaped. Later the same evening a speedboat with five persons wearing camouflage uniforms approached the tug. Two of the persons boarded the tug and took hostage the duty bosun and held him face down at knife point. Seeing the boarding the Master raised the alarm and all crew mustered.
