Safety Violations Halt FV's Voyages
Law enforcement officers from USCGC Mustang terminated an F/V's voyage in Prince William Sound, after they discovered safety violations aboard the vessel. The USCG cited Sheelagh M's captain for several violations, including: Not having immersion suits for the four people aboard his vessel; not having the required number of fire extinguishers; and having a cracked life-ring buoy. Also, enforcement officers from USCG Station Ketchikan terminated vessel Cape Fairweather because of safety violations. Boaters in the downtown Ketchikan area called the USCG to report the vessel being operated negligently. Cape Fairweather is a 49-ft. purse seiner from Ketchikan. The Station Ketchikan boat crew located the F/V at a Ketchikan fuel dock and conducted boarding on it.
Cutter Liberty Terminates Juneau Fishermen for Safety Violation
The voyage of F/V Lady Jane was terminated for a safety violation by USCGC Liberty. The vessel's captain, Auke Bay resident Jacob Hallingstad, and his two crewmen, Trevor Kearney and Lynn Marks, were escorted from Stephen's Passage into Harris Harbor after a boarding team from Liberty discovered the vessel had only one immersion suit aboard. Safety laws require one immersion suit to be aboard for each person sailing on the vessel.
USCG Terminates Voyages
The USCG terminated the voyage of F/V Miss Betty T due to numerous major safety violations southeast of Beaufort Inlet, N.C. A boarding team from USCGC Point Warde boarded the 66-ft. shrimp trawler, discovering the wooden hulled vessel was taking on approximately 450 gallons of water per hour. Other safety deficiencies included inadequate fire fighting equipment, exposed wires throughout the vessel, fuel leaks, missing hatch cover and improper maintenance of lifesaving equipment. The USCG also terminated the voyage of F/V Frisco, due to major safety violations approximately three miles east of Parramore Island, Va. Oil Tanker Detained for Safety Violations
U.S. Coast Guard personnel on Monday detained the oil tanker Overseas Jademar in Port Angeles after several safety violations were discovered during a port state control exam. Marshall Island-flagged tanker Overseas Jademar was en route from Malaysia to Anacortes, Washington to offload cargo when Port State Control officers from Sector Puget Sound found several significant safety violations during a port state control exam. The ship will remain detained in Sector Puget Sound's Captain of the Port zone until the safety violations are corrected. The deficiencies found onboard included nonfunctioning distress signaling equipment and fire safety systems.
Containership Detained in Tacoma for Safety Violations
The 685-foot Liberian-flagged containership Westwood Robson is being held in port after significant safety violations were discovered by Port State Control officers from U.S. Coast Guard Sector Puget Sound during an inspection at the Husky Terminal in the Port of Tacoma, Thursday. The inspection revealed excessive oil in the bilge holding area, oil-soaked lagging present throughout machinery spaces and clogged oil drains, creating a significant risk of fire, the Coast Guard said. Mechanical issues included deteriorated fittings that contributed to oil and coolant leaks as well as inoperable oil purifiers. The vessel and crew must are required to stay in port until the deficiencies have been corrected to the satisfaction of Coast Guard inspectors and the Classification Society.
Coast Guard Cites Vessel Master for Safety Violations
A Coast Guard boarding team cited the master of a commercial fishing vessel with safety violations Tuesday off the coast of Atlantic City, resulting in the termination of the vessel's voyage and restriction to A law enforcement boarding team from the Coast Guard Cutter Ibis boarded the 70-foot commercial fishing vessel Lady Kimberly, fishing for summer flounder, about 63 miles east of Atlantic City. In the course of the boarding, the team found that the vessel lacked a general emergency alarm, a Coast Guard-approved flare and any sound-producing device. These are federally-mandated safety regulations that the Coast Guard routinely checks for when they conduct safety boardings.
US Detains Bulker for Safety Violations
A 738-foot motor vessel Kind Seas was detained by the U.S. Coast Guard after significant safety violations were found during an inspection in Kalama, Washington, Friday. Discrepancies were discovered by Portland’s Coast Guard Marine Safety Unit vessel inspectors during a routine inspection of the 1998-built Marshall Islands-flagged bulk carrier vessel. Safety violations were related to a complete failure of the emergency generator, which provides power to emergency equipment including the emergency firefighting pump system. Other discrepancies include deficient structural fire boundary doors designed to prevent the spread of a fire, inoperable bilge pumps critical to removing excess water and waste oil accumulation in engine compartments, and inoperable life saving communication equipment.
Bulk Carrier Detained in Portland
The U.S. Coast Guard said it detained the 579-foot motor vessel Strategic Synergy, Tuesday, requiring the vessel to remain in Portland until numerous safety violations are corrected by the ship’s crew. The Strategic Synergy, owned by SBC Synergy PTE. LTD., intends to load grain in Portland and will depart for Ecuador after the safety violations have been met. Port State Control Officers from the Coast Guard Marine Safety Unit in Portland discovered the discrepancies during a routine inspection of the newly constructed Singapore-flagged vessel. Fish Tender's Voyage Terminated
USCG law enforcement officers terminated a vessel's voyage for a federal safety violation. Officers from USCGC Naushon boarded Wild Salmon, a 74-ft. fish tender, near Ketchikan. The USCG personnel found Wild Salmon didn't have a survival craft aboard as required by law. The officers ordered Wild Salmon's skipper to take his vessel to Ketchikan and keep it there until he acquired a survival craft.
Costa Concordia – USCG Assists Investigation
U.S. Coast Guard is helping Italian authorities investigate the Costa Concordia disaster. Vice Admiral Brian Salerno told a House Committee the USCG is also on guard against safety violations on foreign flagged vessels that use U.S. ports. Salerno said after the Costa Concordia incident, the Coast Guard immediately offered technical expertise and support to the Government of Italy's marine casualty investigation. He added that the Coast Guard's expertise in marine casualty investigations will prove helpful as the investigation into the disaster moves forward.

Bulk Carrier Detained in Oregon
The U.S. Coast Guard detained the 600-foot bulk carrier vessel Ikan Sudip, after discovering significant environmental and safety violations during an inspection in Astoria, Ore. Saturday. Vessel inspectors from the Coast Guard Marine Safety Unit in Portland discovered the discrepancies during an inspection of the Panamanian-flagged motor vessel, which was built in 2004. “The Ikan Sudip’s safety and environmental conditions posed an unacceptable threat to our waterways," said Capt. Dan Travers, Coast Guard Sector Columbia River Commanding Officer and captain of the port for Oregon and Southern Washington. While inbound to U.S. waters, Monday, the vessel experienced two separate fuel related propulsion losses within 24 hours, which completely disabled the vessel at sea.
Safety Violations Terminates Voyage
Coast Guard law enforcement officials terminated a fishing vessel’s voyage Tuesday afternoon in Southeast Alaska waters after the boat’s captain failed to meet federally required safety equipment regulations. Boarding officers from the Petersburg-based cutter Anacapa boarded the 37-foot vessel Charisma at about 10 a.m. near Takatz Bay in Chatham Strait. The Coast Guard officials cited Charisma’s operator for safety equipment violations that included unserviceable survival suits, an expired life raft, an expired electronic position indicating radio beacon and having no sound-producing device. Containership Lowlands Kamsar has been detained by U.S. Coast Guard personnel in Seattle, Monday, after a Port State Control exam detected several safety violations. The Lowlands Kamsar a 751-foot, Panamanian-flagged ship will remain in Sector Puget Sound's Captain of the Port zone until the violations are corrected, the Coast Guard said. During the exam Coast Guard Port State Control officers discovered that the automatic fire extinguishing system that protects the vessel’s engine room had been disabled by the crew, additionally, the vessel’s owner, Misuga S.A., failed to ensure that appropriate corrective action was taken. The Coast Guard detained the Arctic Transport in Seward Wednesday for five safety violations. Tom Rebar, skipper of the Arctic Transport, informed the Coast Guard Monday that the 203-foot ship was experiencing engine problems. The vessel departed Seattle days earlier and was in the Gulf of Alaska when it experienced engine problems and flooding. USCG Terminates Hazardous Fishing Vessel Voyage
The USCG investigated a fishing vessel after finding numerous safety violations during a routine boarding 50 miles southeast of Cape May, N.J. Six crewmembers of Ruby S, a 72-ft. scalloping vessel homeported in Hampton Roads, Va., were fishing offshore when USCGC Mako from Cape May, boarded the vessel. During the boarding, Mako's boarding team members found numerous discrepancies, including water leaking through the shaft packing at 25 gpm, watertight integrity problems on the main deck, rusted hatches in forward compartments and the engine room, compartment hatches cut out to accommodate air-conditioning units and faulty alarms and flares.
Tanker Captain Charged In San Francisco
The captain of an oil tanker that arrived in the San Francisco Bay with safety violations, including risky oil leaks, was charged on with falsifying documents and lying to Coast Guard inspectors. The Greek captain, who speaks little English, was arrested over the weekend after his Singapore-registered Neptune Dorado was found with oil in the ship's ballast tanks. If ignited, vapors from the oil could have caused the ship to explode, officials said. Ballast tanks are only supposed to hold water, in order to stabilize a ship, but the tanks on Neptune Dorado contained more than 500 tons of crude oil, according to court documents, which called it a "serious safety hazard."

USCG Detains Foreign Freight Vessel Again
USCG inspectors in Dutch Harbor detained 281-ft. freight ship Jacha for the third time since Aug. 1998 due to Safety of Life At Sea violations (SOLAS). Inspectors found seven discrepancies aboard 14-year-old Jacha, including fire doors that didn't close properly, an incompetent fire drill and water leaks on the vessel's main engine. Due to Jacha's history of safety discrepancies, this time, it is not permitted to conduct any cargo operations, bunkering operations or move within the port until some of the discrepancies are corrected to the satisfaction of the Captain of the Port. Flagged in Panama, Jacha is not an unfamiliar site in the port of Dutch Harbor/Unalaska.
Tanker Captain Bail Set At $500K
A U.S. judge set $500,000 bail for a Greek tanker captain charged with falsifying documents and lying to U.S. Coast Guard inspectors after his ship was found in San Francisco Bay with numerous safety violations, including risky oil leaks. U.S. Magistrate Joseph Spero set a Friday hearing to determine conditions for Capt. Kiriakos Daioglou's release if he is able to post bail, local news reports said. Daioglou was also ordered to remain in the United States at least until a bail review hearing next month. Daioglou was arrested over the weekend after USCG inspectors found oil leaks in the Singapore-registered Neptune Dorado's ballast tanks, which could have exploded if ignited. Roughly 177,000 barrels of crude from the tanker had been unloaded at Tosco Corp.'s Rodeo, Calif.
Shell Protesters Cited for Entering Safety Zone
U.S. Coast Guard (USCG) Sector Puget Sound investigating officers initiated civil penalties, Friday, against four individuals who entered an established safety-zone around a Shell-contracted vessel in Bellingham during Memorial Day weekend. Cody Erdman, Chiara D’Angelo, Paul Adler and Matthew Fuller were cited in accordance with Code of Federal Regulations for entry into or staying in a federally-regulated safety zone between May 22 and 24. USCG officials can seek a maximum civil penalty of $40,000 for each entry into the zone or day the individuals violated the zone. The final penalty will be determined by the Coast Guard Hearing Office in Arlington, Va. Hearing officers will be assigned and provide the individuals an opportunity to refute the charges or provide evidence on their behalf.
