The UK Department for Transport issued a News Release stating that the Marine Accident
Investigation Branch (MAIB) published the report of its investigation of the grounding of the M/V LYSFOSS in the Sound of Mull, Scotland on May 7, 2001.
The investigation found that the chief officer was alone on the bridge when the grounding occurred, the helmsman having departed to conduct rounds. No passage plan had been prepared for the transit, contrary to flag state
requirements. The MAIB recommended that the vessel owner review its safety management system, among other things.
